All clients must have an annual good faith exam GFE conducted by a mid level (NP/PA) provider prior to any services provided, if you do not have a current completed GFE, please click the link below to complete GFE forms prior to booking a service.  Your scheduled service/consultation will be rescheduled if the GFE is not completed prior to scheduled time. ​​
