This VIP Membership Agreement ("Agreement") is entered into between Infinity Beauty ("Company") and the undersigned client ("Member"). This document is designed for both printed and digital signature execution.
1. Membership Program
VIP Membership provides exclusive benefits, preferred pricing, promotions, and services as
determined by Infinity Beauty. Benefits may be modified with reasonable notice.
2. Membership Fee & Term
• $99 per month
• Minimum commitment: 6 months
• Automatically billed monthly to the card on file.
3. Payment Authorization
Member authorizes automatic charges to the credit card on file for membership fees, applicable taxes,
cancellation fees, and outstanding balances. A valid card must be maintained at all times.
4. Early Cancellation
This membership is a binding 6-month agreement. If canceled early, Member agrees to pay 65% of the remaining contract balance. The fee will be charged immediately and benefits will terminate.
5. No Refund Policy
All payments are non-refundable once processed.
6. Non-Transferability
Membership benefits may only be used by the enrolled Member.
7. Appointment & Treatment Policies
Services are subject to provider discretion, medical appropriateness, and all Infinity Beauty policies.
8. Governing Law (Texas)
This Agreement shall be governed by the laws of the State of Texas. Venue for disputes shall lie in Texas.
