This Membership Agreement ("Agreement") is entered into between Infinity Beauty ("Company") and the undersigned client ("Member"). This document is designed for both printed and digital signature execution.

1. Infinity Beauty VIP Membership and Infinity Beauty and Beyond Membership Programs

Infinity Beauty VIP Membership provides exclusive benefits, preferred pricing, promotions, and services as determined by Infinity Beauty. Infinity Beauty and Beyond Members provides Infinity VIP benefits with additional products and services as determined by Infinity Beauty. Benefits, products and/or services included with any membership may be modified with reasonable notice.

2. Membership Fee & Term

• Infinity Beauty VIP Membership, $99 per month, Minimum commitment: 6 months

• Infinity Beauty and Beyond Membership, $149 per month, Minimum commitment: 12 months

• Membership plans are automatically billed monthly to the card on file.

3. Payment Authorization

Member authorizes automatic charges to the credit card on file for membership fees, applicable taxes,

cancellation fees, and outstanding balances. A valid card must be maintained at all times.

4. Early Cancellation

The Infinity Beauty VIP Membership is a binding 6-month agreement. If canceled early, Member agrees to pay 65% of the remaining contract balance. The fee will be charged immediately and benefits will terminate.

The Infinity Beauty and Beyond Membership is a binding 12-month agreement. If canceled early, Member agrees to pay 65% of the remaining contract balance. The fee will be charged immediately and benefits will terminate.

5. No Refund Policy

All payments are non-refundable once processed.

6. Non-Transferability

Membership benefits may only be used by the enrolled Member.

7. Appointment & Treatment Policies

Services are subject to provider discretion, medical appropriateness, and all Infinity Beauty policies.

8. Governing Law (Texas)

This Agreement shall be governed by the laws of the State of Texas. Venue for disputes shall lie in Texas.

Credit Card Authorization:

Please complete all fields. You may cancel this authorization at any time by contacting us via email at infinitybeautyntx@gmail.com. The contract cancellation fees will be applied and charged unless a replacemet card is placed on file or contract is paid in full.

This authorization will remain in effect until cancelled in writing via the provided email.

I, ("Member") authorize Infinity Beauty, LLC to charge my credit card  above for agreed upon purchases. I understand that my information will be saved to file for future  transactions on my account.
